Student Information Sheets
Your students safety is one of our main concerns. These information sheets will allow us to have all the information we need in-case of an emergency. The following form will provide us with your students information, your information, emergency contacts, photo release, medical consent, and proper insurance information.
We ask that you check with Brent Moon to make sure that your students information sheets are up to date and properly filled out. You can download the information sheet, print, fill out and return to the church with a copy of the students insurance card attached.
